In 4.2.1 Movement Rates, it says "If any Units of a ployed Formation will pass through rough terrain, all Units of the Formation will suffer the terrain penalty." For Dense Terrain the "entrance and exit cause a halt" - one could interpret this as applying once to the whole Formation. Is this correct?

Yes. A ployed Formation is effectively treated as a single Unit for the purpose of movement.
