If a formation enters rough or dense terrain does the Reformation Area always follow and take up the same space? For example does the Reformation Area block the bridge stopping the formation behind from crossing?

A Reformation Area doesn’t completely block the bridge, as it can be moved through paying the passage-of- lines penalty (4.2.1, 4.2.2).
