Firstly, fatigue picked up during the combat phase. Is it applied as soon as it occurs to the whole of the formation or at the end? It seems a trifle unfair to units at the left side of a combat to suffer because of failures on the right.

Yes, it is immediately applied. The purpose of this is for success to breed success, if one end of the line begins to collapse, the whole line will start to falter.
So yes, this is intentional. Remember that fatigue isn't specific to the Units, it is representing the total condition of the Formation.
