In combat, I assume you recalculate the modifiers as you move from right to left, meaning that fatigue was accumulating and the modifiers on each sides were getting worse.

That is correct, so “success builds on success” and “failure follows failure”.
Generally a player doesn't (and shouldn't) recalculate the modifier from scratch, just make sure to modify it based on what has changed. So, if a Formation accumulates enough fatigue in the last Combat Resolution performed that it will cause a new -1 in this next Combat Resolution, make sure to apply that -1.
