Assume there is an exchange of fire and both sides are still standing at the end of the skirmish phase – then what? Does the attacking formation move again into close combat or does it have to wait until the next move before close combat and after another exchange of skirmish fire?

The attacking [A] Formation will only move during the Movement Phase per 4 (unless mandated to by an Assessment result for instance), so if it doesn’t make contact during Movement in the current turn, it will wait until the following turn’s Movement Phase to move to contact. And yes, Artillery & Skirmishing will happen in both turns, the Artillery & Skirmishing Phase occurs in every turn when there are valid targets (5.1)
